Performance Assessment Form Instructions
- Select the appropriate assessment instrument to use: the Performance Assessment for Non-Supervisory Personnel or the Performance Assessment for Supervisory Personnel (if two or more employees are supervised). The following instructions apply to both assessment forms.
- Complete Section I, Identification.
- Familiarize yourself with the Levels of Performance as defined in Section II. These Levels of Performance are used throughout the Performance Assessment and may not be changed. In other words, you do not have the discretion to invent a new Level of Performance rating such as "2.5".
- In Section III, rate the ten defined performance factors using the Levels of Performance below each criterion. If a specific factor does not apply, check "not applicable".
- After the 10th Performance Factor, there is a "Rating for Section III". This rating is an average of the Performance Levels for Section III. Do not average any Performance Factors where "Not Applicable" was checked.
- In Section IV list the three most Essentials Job Functions of this employee's job. These must be broader than tasks. Assess the employee's performance of these Job Functions using the Levels of Performance as defined in Section II. Average the ratings for Section IV and enter this number in the "Rating for Section IV" section.
- If goals were communicated to the employee, then complete Section V. List only the three most significant goals and rate the employee's progress on these goals using the Levels of Performance. Average the ratings for Section V and enter this number in the "Rating for Section V" section. If no goals were communicated to the employee, be certain to complete Section VI.
- Section VI, Goals, Initiatives, and Priorities for Upcoming Year is an opportunity for you to share with the employee new performance expectations, new projects and changes in priorities for the coming year. Be as specific as possible and remember to update this section if changes occur during the year.
- Section VII, overall Performance Rating and Summary – Average the three average scores in Sections III, IV and V. Carry the number out two places beyond the decimal and enter the exact score by one of the categories in this section.
- Section VIII, Employee's Comments – The form should be signed by the preparer (supervisor) then the reviewer (Department head, Vice President, etc.), then the employee. If the employee wants to make comments, let him/her write as much as necessary. The Performance Assessment cannot be changed once the employee has signed without the employee's knowledge. Be certain to read what the employee has written and take any necessary action. The employee should receive a copy of the completed Performance Assessment.
