New Employee Paperwork
Full-time employees must attend a new hire orientation to complete the paperwork below (contact Human Resources for date, time and place). Please plan to spend a minimum of two hours; time will vary depending on the number of new hires and questions needed to be answered.
Part-time employees need to visit Human Resources for the employment and payroll process.
All employment is contingent on the positive outcome of a background screening check. Background screening forms can be found at the Human Resources Forms Library website.
I. Faculty Full-time and Part-time
Contact the hiring department for employment procedures or:
- The Provost Office for full-time faculty.
- The Associate Provost's Office for part-time faculty.
II. All Faculty and Staff
The following documents are needed from the new hire to comply with Federal government regulations and Samford HR practices:
- A valid passport; or
- Current driver's license & Social Security card; or
- Current driver's license and a notarized/certified birth certificate
- A check to void for direct deposit.
Samford University requires you to complete the following information in order to process your employment paperwork and add you to our payroll:
- Employment Application (Faculty or Staff)
- I-9 (Must be completed on or before the first day of employee's date of hire). Documents must be presented within 3 working days of date of hire.
- A-4 (State tax forms)
- W-4 (Federal tax forms)
- Personal Information Form
- Authorization Agreement for Direct Deposit
- Confidentiality-Security Agreement
- Receipt of Policies
