Effective 9/1/2009, flu shots, both seasonal and H1N1 (when available), will be covered for all participants in the Samford University BCBS plan regardless of place or provider. With that said, this is how the coverage will work for our plan. Please read this carefully to ensure proper procedure.
1. You can go to your primary physician's office to receive the injection. Most offices will let you come in to see just the nurse to receive the shot. However if you see your primary physician during the visit you will be required to pay the $25.00 primary physician co-pay. (Reminder: Primary Physicians include your Internist, Family Care Doctor, Pediatrician, and OBGYN). If you go to the doctor's office they will file the claim directly with BCBS.
2. If you go to a retail outlet (drugstore, WalMart, Publix) you will have to pay up front for the cost of the shot and then file a paper claim with BCBS for reimbursement. The paper claim you will need to use can be found in the forms library. You must have a receipt to submit along with the claim. BCBS will process these claims as soon as administratively possible. Do not expect an immediate turn around on your reimbursement.
3. The shots that will be given by Maxim Healthcare on campus will also be covered for members of the Samford University BCBS plan, assuming Maxim gives you a receipt. I have been told by Student Health that Maxim should be giving receipts. Ask for a receipt prior to receiving your shot. You still must pay for the shot up front and then file for reimbursement with BCBS using the paper claim and your receipt.
