Open Enrollment for 2010 Medical Spending Account, 2010 Dependent Care Spending Account, and BCBS Health Plan:
Open enrollment for the Medical Spending Account, Dependent Care Spending Account, and Samford BCBS Medical Plan begins Monday, November 16 and will continue through 12/18/2009. To sign up for these benefits you will need to take action by December 18th. All benefits will become effective on 1/1/2010.
1. Medical Spending Account 2010 – this plan allows you to contribute your money, before federal taxes (this means you never pay tax on this money), to a spending account which you can then use to reimburse yourself for medically necessary expenses incurred by you or your dependents that are not covered by insurance. Because this is a tax free benefit, the IRS requires participants to re-enroll in this plan each year. This means if you want to participate in this benefit for 2010, you must complete the 2010 Medical Spending Account enrollment form. The form can be found on the Human Resources website under the Forms Library. Please print this form, complete it and submit to Amy Key in Human Resources by December 18th. We must have original signatures, so please do not fax or email the form to HR. In order to make the Medical Spending Account available to cover more of your medical expenses, the maximum contribution you can make to the Medical Spending Account for 2010 has been increased to from $3000 to $5000.
2. Dependent Care Spending Account 2010 – this plan allows you to contribute your money, before federal taxes, to a spending account which you can then use to reimburse yourself for dependent care expenses, such as payment to a daycare or in-home nanny. This account is not for reimbursement for dependent medical expenses. Because this is a tax free benefit, the IRS requires participants to re-enroll in this plan each year. This means if you want to participate in this benefit for 2010, you must complete the 2010 Dependent Care Spending enrollment form. This form can be found on the Human Resources website under the Forms Library. Please print this form, complete it and submit to Amy Key in Human Resources by December 18th. We must have original signatures, so please do not fax or email the form to HR. The maximum annual contribution for 2010 is $5000.
3. Medical Plan – If you are not currently a participant in the Samford University Medical Plan (Blue Cross Blue Shield) you may apply for coverage for you and your dependents at this time to be effective January 1, 2010 (if you are currently a participant and wish to continue, you need take no action at this time). You may also at this time make changes to your current BCBS plan (add an eligible dependent, remove a dependent, cancel coverage) to be effective January 1, 2010. This application cannot be emailed. If you wish to apply or make changes to your current BCBS contract please send an email to Amy Key at aekey@samford.edu to request the BCBS form.

Optional Life Insurance Open Enrollment: Now through November 30, 2009. During this open enrollment period you may enroll in the optional life insurance plan or if currently enrolled you may increase your optional life amount up to the maximum (subject to evidence of insurability).
