DSS Policies: Accommodation Letter

The following policies apply to DSS operations. Registered students should be familiar with those policies that apply to their particular requests. DSS may change, alter, or modify policy.

Accommodation Letter

The following is the official notice of accommodation needs that students’ request from DSS at the beginning of each term and is sent to professors:

This information is confidential.  The above named student has a disability that has been verified by an appropriate professional.  Based on this student’s individual needs, disability documentation, recommendations from health care or educational specialists, and other relevant factors, the following academic adjustments have been approved for this student:

(Accommodations may include the following examples:)

-Extended time on exams (1 ½  or double)
-Extended time on exams if student experiences exacerbation of symptoms
-Reduced distraction testing environment
-Tape recording of lectures
-Note-taker
-Books in alternate format
-Copies of handouts provided in large print
-Enlarged print for exams
-Exams in Braille
-Alternate test format, if needed
-Use of in class assistant
-Ergonomic chair
-Exams at DSS* (optional)
-Scribe (arranged by DSS)
-Excused absences caused by disability
-Provision to make up missed exams and/or assignments

-Preferential seating (front, back, near exit, end of table)
-Provision to leave seat briefly if necessary
-Sign language interpreter (arranged by DSS)
-Eat and drink in class, if needed
-Alternative evaluation criterion for oral class presentation
-Provision to stand and sit as needed
-Exams at DSS because student needs adaptive technology
-Student may be tardy to class due to disability
-Provision to leave class briefly to use restroom (even during exams)
-Provision to take breaks during exams
-Use of a spell checker (even during exams)
-Use of a calculator (even during exams)
-Priority Registration

Your professors have been sent the above letter.  You should introduce yourself and meet with them to make any arrangements for your accommodations.  If you encounter any problems with your accommodations please contact Disability Support Services, located on the first level of Pittman Hall # 726-4078 or #726-2105.
